Introducing CDR in InstaNote: Documentation Review, Before Sign-Off

InstaNote now includes CDR, a clinical documentation review capability that reads the draft note before the clinician signs. It surfaces documentation opportunities while the encounter is still fresh, so the record is more complete before it reaches coding, with the clinician in control of every note.

Instnote now includes CDR, a clinical documentation review capability that works inside the note. CDR reviews the draft note for documentation opportunities before the clinician signs, so the record is more complete before it moves into coding. The clinician stays in control of every note and every code suggestion.

    Review that Happens Before Sign-Off

CDR reviews the draft note rather than interrupting dictation. After the encounter is captured and CDR generates a draft note, CDR reads it and surfaces documentation opportunities for the clinician to consider. Because this happens before the note is finalized and before billing, CDR addresses gaps while they are still easy to correct, rather than surfacing them later as queries or denials.

    What CDR Reviews

CDR looks at the draft across four areas. Completeness flags where treatment, procedures, or prescriptions may need clearer support. Consistency flags mismatches in the note. Specificity encourages adding detail to support a clearer record. Quality support surfaces items tied to important care and quality measures when they are relevant to the encounter.

These checks are supported by validation against established frameworks, including HCC risk adjustment, MEAT and TAMPER documentation criteria, and HEDIS quality measures. CDR can also be configured to review the requirements a customer prioritizes, where the data supports it.

    The Clinician Stays in Control

CDR surfaces gaps and brings them to the provider's attention. It does not act on its own. The clinician reviews the item highlighted and updates the note as appropriate. After the clinician completes the note updates, the note can be finalized based on the chosen workflow. The final content and sign-off always stay with the clinician, which keeps documentation defensible and clinical judgment where it belongs.

    What CDR Means for Clinicians

For clinicians, CDR is designed to reduce post-note work. Feedback appears on the draft while the encounter is still fresh, so a gap becomes a quick edit rather than a query days later. Seeing documentation gaps before sign-off means fewer clarifications and less back-and-forth once the note has moved on. Because CDR lives in the same InstaNote experience clinicians already use, there is nothing separate to open and no change to how they document. The clinician reviews, decides, and signs, and CDR makes the gaps easier to see while there is still time to act.

What CDR Means for Revenue-Cycle and Quality Leaders

For revenue-cycle and quality leaders, CDR strengthens the record at its source. Documentation is where the revenue cycle starts, so a more complete note gives coding and quality teams a cleaner foundation to work from, with less effort spent correcting documentation after the fact. Cleaner source documentation supports more accurate coding and more consistent reporting, and it reduces downstream queries that slow the back end.

For teams working toward value-based care, CDR surfaces quality-support items, including those tied to HEDIS measures, while the record can still be improved, supporting quality capture without adding a separate review step. In each case, CDR is designed to make the documentation that feeds coding and quality more dependable.

    How CDR fits InstaNote

CDR is a feature of InstaNote, the AI documentation experience clinicians already use for ambient and dictation. It adds a review step to the organization's existing workflow, so it stays in the same platform and window as the note draft. CDR strengthens the note without adding to the clinician's day.
